Each dot is a Primary Health Centre from the Ministry of Health & Family Welfare's All India Health Centres Directory (as of 7 October 2016): 28,110 PHCs with usable coordinates.

• The data is from 2016. Some PHCs have since been renamed Ayushman Arogya Mandirs, upgraded, or closed, and newer ones are missing.
• Some pins are approximate (orange ring). They may mark a village or block centre, or a nearby facility. Search by name to find the building.
• District names are as of 2016. Many districts have since been split.
• Cleaning: duplicate "Andhra Pradesh Old" rows removed; swapped latitude/longitude corrected.
• Facilities the general public cannot use are hidden: CGHS and ESI dispensaries, police, jail and armed-forces dispensaries, railway hospitals, and dispensaries inside high court or secretariat complexes.
